How Physician wRVU Compensation Works
Work Relative Value Units (wRVUs) are the foundation of most physician productivity-based compensation models used by hospitals and medical groups across the United States. Each CPT code (procedure or visit type) is assigned a specific wRVU value by the Centers for Medicare & Medicaid Services (CMS) as part of the Medicare Physician Fee Schedule.
The Conversion Factor
Your employer multiplies your total annual wRVUs by a dollar-per-RVU conversion factor to determine your productivity income. For 2024–2025, the Medicare conversion factor is approximately $32–$33/RVU, but employer conversion factors are typically much higher ($45–$60+/RVU) to account for overhead and market compensation rates.
Blended wRVU Per Visit
Because physicians see a mix of new patients, established patients, and procedures—each with different CPT codes and different wRVU values—your "blended wRVU per visit" represents the weighted average across your full patient mix. A new patient visit (99205) carries 3.50 wRVUs while a simple established patient visit (99213) carries 1.30 wRVUs.
Bonus Components
Most modern physician compensation models blend wRVU productivity with quality metrics, patient satisfaction scores, mission-related activities, and panel management. These bonus components are typically expressed as a percentage of your total compensation package and are subject to annual performance reviews.
